What Is Weld Failure Analysis?
Weld failure analysis is the systematic engineering investigation of a cracked, leaked, or ruptured weld to determine the failure mechanism, root cause, and contributing factors so that effective corrective action can be developed to prevent recurrence. The analysis combines visual examination, metallurgical examination of the failed weld and surrounding material, review of the original welding procedure and inspection records, evaluation of operating conditions and load history, and comparison against the applicable fabrication and design codes to establish why the weld failed and what needs to change.
Weld failures in process piping and equipment can result from fabrication defects, inadequate welding procedures, improper heat treatment, service-induced cracking mechanisms such as stress corrosion cracking, fatigue from thermal cycling or vibration, or operating conditions including nozzle overload that exceeded the weld’s design basis, with accurate root cause identification essential to selecting the right corrective approach, potentially including fitness for service evaluation of the affected area.
Applications
In-Service Weld Cracking Investigation
When cracking is discovered at a weld during operation or inspection, failure analysis determines the cracking mechanism and whether the condition is isolated or indicates a systemic issue affecting similar welds throughout the facility.
Fabrication Defect Disposition
Weld defects discovered during fabrication inspection require engineering evaluation to determine whether they can be accepted as-is through fitness for service assessment, need repair, or require complete weld removal and re-welding.
Recurring Failure Prevention
When the same weld location fails repeatedly after repair, failure analysis investigates whether the root cause is the welding repair procedure itself, the operating loads at that location, or an underlying design issue that repair alone cannot resolve.
Benefits
Identifies the actual failure mechanism rather than guessing. Proper failure analysis distinguishes between fatigue, corrosion cracking, hydrogen cracking, lack of fusion, and other mechanisms that require fundamentally different corrective approaches.
Prevents recurrence through targeted corrective action. Corrective action based on confirmed root cause addresses the actual problem, while action based on assumption may address the wrong mechanism and allow the real cause to produce additional failures.
Determines whether the failure is isolated or systemic. Establishing whether the failure mechanism could affect other welds in similar service across the facility is critical to determining the scope of inspection and corrective action needed.
Limitations
Quality of analysis depends on evidence preservation. Cutting, grinding, or repairing a failed weld before metallurgical examination destroys the evidence needed for accurate failure analysis, making evidence preservation procedures essential.
Complete analysis requires multiple disciplines. Thorough weld failure analysis may require metallurgical expertise, piping stress analysis to evaluate operating loads, and process engineering input on operating conditions and upset history.
Root cause may involve multiple contributing factors. Weld failures often result from a combination of factors, such as a minor fabrication defect combined with high cyclic loading, where neither factor alone would have caused failure.
FAQ
How does weld failure analysis relate to mechanical integrity programs?
Weld failure findings feed into the mechanical integrity program by identifying degradation mechanisms that the inspection program should target at similar welds across the facility.
How does weld failure analysis relate to piping vibration?
Piping vibration is a leading cause of weld fatigue failures, and failure analysis that identifies vibration fatigue as the mechanism triggers vibration assessment and corrective action on the affected piping system.
How does weld failure analysis relate to process safety?
Weld failures resulting in loss of containment are investigated under process safety management incident investigation requirements, with findings driving corrective actions to prevent recurrence.
